The, In My Own Words™, advance directive video service started development in
2005. Created by Dr Melinda Ginne, drmelindaginne.com, this service allows anyone
18 years of age and older, to video record their wishes for their care in the event
that they become incapacitated.
A video of you, stating your health care preferences, including the types of special
treatment you want or don't want at the end of life, your desire for diagnostic
testing, surgical procedures, cardiopulmonary resuscitation and organ donation will
give the support and strength to your family and friends at this most important
point in your life.
This recording allows people to specify what kind of unique care they will want, for
example to have their pet visit them, or have someone read them poetry, or massage
their feet. The video version does not replace a written version but enhances the
written form.
The video version can be stored on a flash drive which can easily be
carried on a key chain. At your request, copies can be sent to your physician,
attorney, your designated health care agent, or your family.
An advance health care directive lets your physician, family and
friends know your health care preferences, including the types of
special treatment you want or don't want at the end of life, your
desire for diagnostic testing, surgical procedures, cardiopulmonary
resuscitation and organ donation.

The most common written advance directive in California is the version developed by the California Medical Association.
An advance health care directive is the best way to make sure that your
health care wishes are known and considered if for any reason you are unable
to speak for yourself or are terminally ill. In addition, an Advance Health Care
Directive allows you to appoint someone you trust to speak for you when you
are incapacitated.
