Why a video advance health care directive?
For your Loved Ones
A video of you, stating your health care preferences, including the types of special treatment you want or don't want at the end of life, your desire for diagnostic testing, surgical procedures, cardiopulmonary resuscitation and organ donation will give the support and strength to your family and friends at this most important point in your life.
You can specify personal requests such as the music you would like to hear played, people or pets you would like to visit you, as well as any other wishes for your comfort. You can also include final wishes or messages for your family and or friends.
Most importantly, the video will provide comfort, strength and support to your loved ones. It will clarify your health care decisions for your designated Health Care Agent who will carry out your wishes and be your advocate with friends, family and health care providers.
What is an advance health care directive?
An Advance Directive is a legal document describing your medical wishes in the event that you cannot advocate for yourself. It addresses use of life support systems such as a ventilator, feeding tube, dialysis, CPR, surgery, organ donation, blood transfusion, and medications. The most common written advance directive is issued by the California Medical Association (CMA). |
